• Contact Information  (required at first session)

  • ​This provides me with the best means to reach you, and your emergency contacts

• Informed Consent​ (required at first session)

  • This form​ protects your confidentiality and indicates that you voluntarily agree to enter counseling.

​

• Informed Consent for Minors (required prior to first session)

  • This form is required for all clients who are under the age of 18, and must be signed by the client's parent(s) or legal guardian.​​​

​

• Informed Assent of Minors (required at first session)

  • This provides me with a minor's informed assent to enter counseling, complementary to the Informed Consent for Minors form.​
